    18/00041 - Community Navigation Service (Care Navigation and Social Prescribing)

    Decision:

     

    To tender for a new contract to deliver a community navigation service for older people and people living with dementia that supports people to navigate the health and social care systems and connects them to community-based support, supporting the demand management for both systems. (The contract will also include carers assessment services) and establish interim arrangements from 1 April 2019 for a minimum period of nine months for the majority of the remaining historic grants to allow for the full tender or appropriate process by 2020 in relation to community-based wellbeing services.

     

    The proposed new contract directly relates to the Council’s strategic outcomes regarding older people and vulnerable residents to ensure they are safe and supported with choices to live independently and underpins the Promoting Wellbeing element of the Council’s Your Life Your Wellbeing Strategy. The new contract will provide information and advice, sign posting and help people find the support they require and connect people to their communities. It will also include a carers assessment. Establishing interim arrangements for the remaining grants will ensure services continue whilst a tender or other appropriate process for a Community-Based Wellbeing Service is undertaken.

     

    Care navigation and social prescribing are a key element of the Sustainability and Transformation Plan (STP) and Local Care Models.

    This matter was discussed by the Strategic Commissioning Board on 23 July and the Adult Social Care Cabinet Committee on 27 September 2018.

     

    There has been extensive engagement undertaken with a range of stakeholders in relation to this project including CCG commissioners and local care leads, Patient and Public Advisory Group (PPAG), district councils and a market engagement event. Engagement focussed on defining the outcomes of the service, clarifying terms, defining the role, agreeing scope and timelines for support and discussing what a future contract might look like

    Financial implications: The contract is expected to have an annual value to adult social care of £3,325,495 (£13,301,980 over the course of a 4 year contract). This assumes that the investment to save options is action and investment in the service is increased in comparison to current spending. This funding is currently invested in annual grants to voluntary sector providers. A number of these will be ended in order for the contract to be put in place. However, it is possible that investment from public health and Clinical Commissioning Groups may increase the value of the contract. Adult social care may also seek to increase the value of the contract over it’s lifetime, by adding additional funding in to the contract.

    Legal implications: TUPE may apply and legal advise will be sought.

    Equalities implications: Equality Impact Assessment has identified a high impact on older people. Adverse impact include the ending of current funded services and the impact on current providers balanced by a positive impact of investing more in the service enabling more people to receive roles and aligning roles more closely to health hubs and district locality teams.
